Use this practical checklist to organise the foundations of a new regulated care service before submitting a CQC application.
Business and service model
Confirm the legal entity, regulated activity, service-user group, operating area, address and realistic financial plan. Make sure the proposed model is clearly understood by every person named in the application.
Leadership and suitability
Identify the nominated individual and registered manager where required. Gather employment history, qualifications, DBS information, references and evidence of relevant skills and experience.
Core documents
Prepare the statement of purpose, safeguarding, medicines, infection prevention, complaints, recruitment, training, governance, data protection, business continuity and other service-relevant documents.
Operational systems
Decide how assessments, care plans, risk assessments, incidents, audits, supervision, training and complaints will be recorded and reviewed. Be able to explain the system, not only show a policy.
Final consistency check
Check names, addresses, regulated activities, aims and contact details across every document. Review all answers for accuracy and keep a complete copy of the submission.

This checklist is a starting point, not an exhaustive regulatory assessment. Requirements depend on the provider and regulated activity.
